Anti-inflammatory effect of anakinra, an interleukin-1 receptor inhibitor, in patients with refractory tophaceous gout

چکیده
The first line treatments for acute gouty arthritis are nonsteroidal anti-inflammatory drugs (NSAID), corticosteroids, and colchicine. However, these agents ineffective or contraindicated in a proportion of patients. Interleukin (IL)-1 is key mediator the pathogenesis arthritis. Recently, three IL-1-targeted have been developed, including anakinra, IL-1 receptor antagonist. authors present two patients with flare refractory tophaceous gout who were treated anakinra.

Chronic tophaceous gout in patients with psoriasis*
Psoriasis is a chronic inflammatory disease of multifactorial etiology influenced by genetic, immunological, and environmental factors. We report the case of a patient with psoriasis for more than 25 years who developed hyperuricemia and chronic tophaceous gout with unusual appearance. In psoriasis, hyperuricemia may occur by increased epidermal cell turnover, which accelerates purine metabolis...
متن کاملManagement of treatment resistant inflammation of acute on chronic tophaceous gout with anakinra.
W e report a 74-year-old man with acute on chronic tophaceous gout in whom conventional treatments failed but who responded to treatment with the interleukin-1 receptor antagonist, anakinra. The patient presented in February 2004 with a severe flare of gout. Multiple joints were swollen, including the right fifth proximal interphalangeal (PIP), the left fourth PIP, and the first metatarsophalan...

Epilepsy is a neurological disease of the central nervous system. It is estimated that about 50-70 million people worldwide suffer from this chronic disorder and 20 to 30% are resistant to conventional anti-epileptic drugs. In the epilepsy therapeutic arena, there is real need for developing novel antiepileptogenesis treatments that offer a way to prevent the onset or the progression of the dis...
متن کاملSparing effect of hemiplegia on tophaceous gout.
The sparing effect of hemiplegia on the development of tophaceous gout is described. The useless upper limb had no tophaceous deposits and the partially paralysed lower limb had only limited urate deposits. Disuse was presumably the major contributor to the limited deposition of urates on the paralysed side.
متن کاملTophaceous Gout Mimicking an Edematous Anconeus Epitrochlearis
We report the case of a 31-year-old male whose initial imaging presentation was considered highly suspicious for an edematous accessory muscle, the anconeus epitrochlearis. Operative excision led to the diagnosis of tophaceous gout. If this etiology had been considered earlier in the patient's clinical course, his diagnostic evaluation may have been less involved. When faced with a soft tissue ...
